Social work is provided as part of SIGMA Home Health Care’s services to address the significant social and emotional impact an illness can have on a patient and their family. The Medical Social Worker assists and counsels patients and families with financial, social and emotional concerns. Social Workers aid patients and their families with personal and environmental difficulties which interfere with obtaining maximum benefits from home health care. Each patient will receive a complete psychosocial assessment and individualized plan of care. Social work services include assisting the patient and family when social, psychological, or economic factors interfere with their care and treatment or inhibit the patient and/or family from coping with disease-induced limitations.
