SIGMA Home Health Care is a team of committed, caring individuals in Bucks, Chester, Delaware, Montgomery and Philadelphia Counties, with our sole focus on providing quality individualized medical, physical and social patient and family-centered services in the comfort of the individual’s home. SIGMA Home Health Care is committed to helping our patients manage their illnesses and/or disabilities while preserving as much dignity as their condition permits and maximizing their quality of life.

Our Vision

It is the vision of SIGMA Home Health Care to provide excellent and safe home health care services that promote the patient’s self-reliance, sense of well-being, and a return to an optimal level of functioning in the familiar surroundings of his/her home. This is achieved through:

• The provision of skilled patient centered care.
• Inclusion of patients and/or caregivers in care planning and implementation.
• Use of a multidisciplinary health care team to effectively collaborate, communicate and coordinate patient care.

Philosophy

SIGMA Home Health Care was established with the primary goal of providing excellent home health care. Our quality, professional, compassionate home health care staff will use their professional expertise to establish and maintain a home health care organization for the care of patients in their homes who are suffering from illnesses or disabilities, or require skilled assessment and teaching, without discrimination based on religious beliefs, race, national origin, age, sex, sexual orientation, disability, or economic status. It is also our philosophy to include both the patient and family to the fullest extent possible, based on their abilities, in their care planning and implementation.

As a member of the health care community, SIGMA Home Health Care works in cooperation with other community agencies to meet the ever-changing health care needs within the communities we serve. SIGMA Home Health Care administration and staff are committed, by our presence and exceptional services, to maximizing the health and well-being of the populous within the communities we serve.
