SIGMA Home Health Care provides skilled nursing services as ordered by the patient’s attending physician, under the supervision of a Registered Nurse (RN) and in accordance with the patient’s plan of care. Our nurses perform a full assessment and develop a care plan based on the needs of patients and their families. All patients and families are encouraged to be a part of planning their care. Patients progress will be evaluated regularly, and prompt action will be taken when any change in the patient’s condition is noted or reported. Our nurses are trained to identify patient and family needs and to provide referrals for other home health services when needed.
